You can use this form to register a child in the 1000 Books Before School program. You must have a library membership in order to participate in this program. You can find out how to sign up for a library membership on our membership page.

It takes about 5 minutes to complete this form.

You will need to:
- tell us your name, email address and phone number
- tell us the name and date of birth of the child you are registering
- tell us your 14 digit library membership number
- tell us your preferred local library
After you submit this form, we will:
- send you an email confirming we have received your registration
- send you details on where to pick up your 1000 Books Before School registration pack

We record your information on our customer database and make it available to relevant Council staff in line with City of Boroondara’s Privacy Statement.
