Boroondara Citizen of the Year Awards 2025 Nomination Form

You can use this form to

Nominate someone other than yourself for either a Boroondara Citizen of the Year award or Boroondara Young Citizen of the Year award.

The awards are split into 2 categories:

• Boroondara Citizen of the Year (26 years or older on 26 January 2025).
• Boroondara Young Citizen of the Year (under 26 years on 26 January 2025).

Before you begin

Check that the nominee is eligible for the award. To read the eligibility criteria visit Boroondara Citizen of the Year Awards.

Nominations can be for a single achievement or for a contribution over many years.

To complete this form

You will need to:

• Tell us your nominee’s name and if they will be under 26 years of age on 26 January 2025.
• Tell us why you’re nominating them and how they’ve made an exceptional contribution to Boroondara in 250 words or less.
• Provide supporting documents, if relevant.
• Tell us your name, email address and phone number.

After you submit this form

We will:

• Send you a confirmation email when we’ve received your nomination.
• Contact you if the judging panel needs more information about the nominee.
• Score your nominee against the selection criteria. Scoring is then discussed before a final decision is made.

What happens with your information?

We record your information on our customer database and make it available to relevant Council staff in line with City of Boroondara’s Privacy Statement.

The contact details you provide to Council may be used by Council to contact you in relation to other Council functions and services.
