Property owners or someone acting on their behalf.

If you are not the current owner of the property, you will need to provide proof that you have authority to apply for this information from the property owner.

To complete this form, you’ll need to provide:

• Relevant Building Surveyor’s name and registration number.
• Form 6 determination issued by a Relevant Building Surveyor.
• A copy of your building permit application made to your building surveyor (Form 1).

Please note we only provide the name and address of adjoining property owners.
